Prevalence of Blastocystis hominis in Patients with Type II Diabetes Mellitus in Kirkuk City, Iraq

Background: Blastocystis hominis is a prevalent human and animal bowel protozoan parasite, which is the most widespread eukaryotic organism in the human intestines. Intestinal parasite usually infects immunocompromised individuals; one of these is type II diabetic mellitus (TIIDM). Aim: The aim of the study was to assess the rate of infection with B. hominis in patients suffering from TIIDM. Materials and Methods: The current study is conducted on 50 participants (20 males and 30 females) (27 diabetic and 23 nondiabetic [control]), aged 25–75 years, diagnosed by a consultant physician, according to the laboratory diagnosis hemoglobin A1c (HbA1c), from November 2021 to March 2022. Two milliliters of venous blood was collected for the evaluation of the ratio of HbA1c with a sample of stool for the diagnosis of the presence or absence of B. hominis. Results: About the age, the highest age group in the TIIDM was 61–75 years, whereas in the control, the highest age group was 46–60 years. About education, 17 samples were from noneducated patients in the TIIDM group and 13 samples in the control group. About residence, 16 were rural residences in the TIIDM group and 23 urban samples in the control group. About autoimmune, 14 samples of diabetic patients infected with another autoimmune disease and 20 samples were not infected in the control group. About B. hominis, all diabetic patients were infected with B. hominis, but in the control group, just two persons have B. hominis infection. Conclusions: B. hominis was more common in the patients with diabetics than the controls. To better understand the relationship between possible risk factors and Blastocystis infection, the sample size must be increased, and the survey area must be expanded.

Background: Irritable bowel syndrome (IBS) is a chronic functional gastrointestinal (GI) disorder with an estimated prevalence of 10% around the globe. Human body possesses about 39 trillion of microbial cells, the majority of which inhabit the GI tract. The recent realization is that small intestinal bacterial overgrowth may be associated with symptoms of IBS. Aim: The current work aimed at the isolation and identification of different bacteria strains from IBS patients. Materials and Methods: One hundred and thirty patients of different sex and ages were diagnosed with IBS (which was previously diagnosed by a specialist doctor) in the time period from November 2021 to May 2022. Results: The current results showed that there is a correlation between the number and percentage of isolates for each type of bacteria with a group of study factors, where in terms of sex, females were more. Concerning the age group, the most common age group was 36–55 years. On the other hand, the percentage of uneducated people was higher in the current study. It also showed the other factor, which is the possession of domestic animals; the majority of the study population was not owning animals. Regarding the prevalent weight, the body mass index (BMI) of 24–29.9 is the highest. The percentage of nonsmokers was also the highest, and the highest percentage of isolates was for Escherichia coli in all. Conclusions: This study proved that E. coli is the most common type of bacteria that has an effect on IBS patients and that Helicobacter pylori has the least effect.

Background: Blastocystis sp. is the most prevalent intestinal protozoa found worldwide in both humans and animals. Objectives: This study aims to identify the subtypes of Blastocystis sp. parasite isolated from diabetes mellitus patients using molecular techniques such as polymerase chain reaction (PCR), DNA sequencing, and phylogenetic analysis. Materials and Methods: The current study was conducted on samples collected from a total of 300 patients (150 diabetic patients and 150 non-diabetic patients). The samples were taken between November 2022 and May 2023 at the laboratories of the General Balad Hospital in Balad City, Saladin, Iraq. Blood and stool samples were taken from each patient, and the investigation included microscopic examination and culturing of the samples on Modified Jones’ Medium for parasitic identification; active infection was confirmed genetically using PCR, and subtyping was done via sequencing of the SSU rRNA gene. Results: Microscopically, only 80 diabetic patients (50 males and 30 females) were infected with Blastocystis hominis, and only 50 people (13 males and 37 females) among the healthy participants had B. hominis infection. Thirty samples were further studied using PCR, and only 16 showed clear bands on the gel, whereas only eight samples were successfully sequenced. Conclusion: This study demonstrated that diabetic mellitus patients (DM group) had a higher prevalence of B. hominis infection compared to the non-DM group. Furthermore, there is a link between the presence of Blastocystis and other risk factors, such as place of residence, water source, and keeping animals, which suggests that exposure to animal stools may result in the development of Blastocystis ST1 among the studied participants.

Background: The protozoan parasite Toxoplasma gondii is responsible for the most significant zoonotic illness in the world, Toxoplasmosis. It is possible for T. gondii to be transferred transplacentally from the mother to the fetus if it is contracted during pregnancy and causes abortion or congenital deformity. Objective: To investigate the correlation between polycystic ovarian syndrome and toxoplasmosis, as well as to examine certain related sex hormones, luteinizing hormone (LH) and follicle stimulating hormone (FSH). Materials and Methods: The current study was conducted at Kirkuk General Hospital and Gynaecological and Pediatric Hospital in Kirkuk city, Iraq, between November 1, 2022 and March 21, 2023. The study included 200 blood samples collected from women who had abortions. The serum used to measure levels of LH and FSH. Results: Out of the 200 aborted women, only 40 (20%) of them were PCO positive (poly cystic ovarian syndrome). The 40 positive PCO subjects were divided into four groups according to the presence of T. gondii antibodies: PCO +ve and Toxo IgM +ve (7.5%), PCO +ve and Toxo IgG +ve (15%), PCO +ve and Toxo IgM ‐ve (42.5%), PCO +ve and Toxo IgG ‐ve (35%). Conclusion: There were significant differences in the FSH and LH level in the study groups. The level of LH in four groups the highest mean was in Toxo IgM and PCO +ve (9.190 ± 3.710) with P value 0.046. In the level of FSH in four groups the highest mean was in Toxo IgG +ve and PCO +ve (8.776 ± 2.061) and the lowest mean was in Toxo IgM +ve and PCO +ve (5.790 ± 3.25) with a P value of 0.045.

BAFF as a Potential Marker for Toxoplasma Gondii Seroprevalence Among Cancer Patients on Chemotherapy

Background: Toxoplasmosis is a global zoonotic disease, that is mainly asymptomatic in healthy persons, but critical and life-threatening in immunocompromised patients. The infection is caused by an opportunistic obligate intracellular parasite protozoon “Toxoplasma gondii” (T. gondii) that can cause fatal complications in immunosuppressed patients. Cancer patients are being immunocompromised from the cancer itself and on the other hand cancer therapy they receive may reactive a latent T. gondii infection that leaves them at high risk. The B-cell activating Factor (BAFF) is a cytokine that plays an essential role in the activation, maturation, and survival of B cells. It is vital for the appropriate functioning of the immune system. Monitoring BAFF levels in cancer patients undergoing chemotherapy could be important for preventing or controlling infections in these vulnerable patients. Objectives: This study aimed to detect seroprevalence of T. gondii in cancer patients receiving chemotherapy and assess the levels of BAFF in those patients. Materials and Methods: 150 serum samples were collected from oncology patients having different types of cancer and receiving chemotherapy, from different ages. And 30 samples from healthy people for control. In this cross-sectional study, samples were collected from the 1st of October 2023 to the 1st of March 2024, at Kirkuk Oncology and Hematology center. The method used was the Sandwich Enzyme-Linked Immunosorbent Assay (Sandwich-ELISA) technique. Results: the overall anti-T. gondii IgG and IgM seropositive samples were found to be 67 (37.2%) in the total study population and 67(44.7%) in cancer patients. BAFF levels were elevated in cancer patients infected with T. gondii compared with the cancer patients with no infection and the healthy control group. A significant difference was indicated with (p-value 0.0004). Conclusions: A high rate of T. gondii seroprevalence was revealed among oncology patients receiving chemotherapy with elevated levels of BAFF associated with T. gondii infection in those patients.

Vulvovaginitis is one of the commonest reproductive tract infections in women worldwide. There are three types of infectious :vaginitis including, bacterial vaginosis ,valvovaginal candidiasis, and trichomoniasis. During the period from December 2021 to April 2022 in the Women conduct Children Hospital in Al-Nasr neighborhood and in private outpatient clinics in Kirkuk city in Iraq. The number of specimens examined was 160 samples(106from healthy and 54from infected women) and the ages ranged between (14-50) years. After the pairs swabs were taken, the direct examination and staining and culturing on the different culture media to diagnose many pathogenic microorganism (bacteria spp, candida spp, trichomonas vaginalis parasite). Trichomoniasis was detected by wet and stained smears and culture . Bacterial infection was confirmed by Nugent scoring method after Gram staining and culture . Candidiasis was identified by direct examination and culturing on both Sabouraud dextrose agar and Chromagar medium. The bacterial and candida diagnosis conforming by vitex-2 system. From the54 sample positive, the commonest organisms was bacteria (33.75%gram negative and gram positive 31.25%), followed by Candida species 14.37%, Trichomniasis as 5%, and mixed infection with these three agents in 17.5%. the commonest organisms was bacteria (100%gram negative and 92.59%), followed by Candida species 42.59%, Trichomniasis as 14.81%, and mixed infection with these three agents in 51.85%.E. coli 15(27.77%) and staph. aureus 16(32%) was the major bacteria isolate and candida albicans(13.12%) was the major of candida spp. isolate. And the results of the relationship of Vulvovaginitis with the age showed that incidence of infection was high among females age group(36-45years) 20(12.5%) . This study conclude that, bacterial vaginosis compile high percentage among women with vaginal discharge. It can be present alone or along with candidiasis while rare cases record a combined infection of bacterial vaginosis along with trichomoniasis and both candidiasis and trichomoniasis may be as secondary infection that enhanced after bacterial vaginosis.

Toxoplasma gondii, a parasite that can produce a wide range of clinical symptoms in humans, is the source of the zoonotic infection known as Toxoplasmosis. The goal of this study was to measure the serum levels of anticardiolipin (ACL) antibodies (ACL-IgM and ACL-IgG) in 200 women with T.gondii and recurrent abortions. Two hundred (200) blood samples from aborted women were used in this study, which was carried out in Kirkuk between November 1, 2022, and March 21, 2023, at the Gynaecological & Paediatric Hospital and Kirkuk General Hospital to determine the level of Anticardiolipin IgG and IgM serum. In this study of 200 aborted women, 28 (14%) had IgM positivity, 66 (33%) had IgG positivity, and 94 (47%) had Toxoplasmosis positivity overall. Of 200 aborted women, 39 (19.5%) had anticardiolipin IgM while 35 (17.5%) had Anticardiolipin IgG. The anticardiolipin IgM higher than anticardiolioin IgG in the Toxoplasmosis infected women.

Blastocystis sp. is anaerobic,global, eukaryotic, found in the human gastrointestinal tract. The purpose of the current study is to estimate the prevalence of B. hominis infection in people with diabetes mellitus. This study is conducted on a total of three hundred (300) individuals, ranging between 4 and 70 years, included (150) diabetic group and (150) non diabetic group. Samples are taken between November 2022 and May 2023. The practical portion and the collection of samples have been done in laboratories of General Balad Hospital in Balad City; Salahadin; Iraq. The study include the collection of 2 ml of venous blood, which is used to measure the HbA1c ratio with stool sample. This is used to examine the presence or absence of B.hominis by microscopic examination after it had been cultured. Modified Jones' Medium has reportedly been used for Blastocystis sp. in vitro culture. Only 80 diabetic patients (50 male, 30 female) were infected with B.hominis after the laboratory diagnosis, and only 50 people (13 male, 37 female) among the healthy participants have B.hominis. This study demonstrate that the DM group had a higher prevalence of this protozoan than the non-DM group. Also, it notice that there is a link between the presence of Blastocystis and other participant characteristics (risk factors). Most results in the present study are display a highly significant difference between both genders, residence, source of drinking water and keeping animals (p<0.05),but there is no statistically correlation with infection by this parasite among the classes of age and educational level (p>0.05) .

Docking of Vaginal Lactobacillus isolates as a Potential Bactericidal Agent in Kirkuk 2022

Background: Because the female genital tract is a complex microbial community that plays various roles in disease development. Alternatively, Lactobacilli in the vaginal environment play an important role in maintaining a healthy vaginal ecosystem by preventing pathogenic overgrowth. Objective: Therefore, this study was suggested to isolate vaginal Lactobacilli with antibacterial activity. Materials and Methods: Two hundred and sixty women participated in this study aged between 15 and 50 years divided into two cohorts of bacterial vaginosis (BV): infected and healthy individuals. BV-isolates isolated and identified according to microbiological and biochemical assays. Accordingly, Lactobacillus isolates were further identified through genomic DNA sequencing of the 16S rRNA, and the results were statistically analyzed. Results: A significant increase (P < 0.05) in the BV-infected women in the age range of 21–30 and 31–40 was recorded, with the percentages of 40.62% and 31.88%, respectively. Alternatively, a significant decrease (P > 0.05) in the percentages of vaginal Lactobacillus species between women of Cohort 1 and Cohort 2 recorded the ratios of 3.125% and 31% validating Lactobacilli presence as a sign of a healthy vaginal environment. Accordingly, four Lactobacillus isolates were assigned for their broad spectrum of antibacterial activity against several pathogenic bacteria. Moreover, results of the partial DNA sequencing revealed the isolation of four Limosilactobacillus fermentum strains registered at the National Center for Biotechnology Information (NCBI) under specific gene bank accession numbers. Conclusion: Effective Lactobacillus species with potential bactericidal activity validates vaginal health.

Molecular Detection of Entamoeba gingivalis among Periodontitis and Gingivitis Patients by Singleplex Polymerase Chain Reaction

Background: Periodontal disease is classically characterized by progressive destruction of the soft and hard tissues of the periodontal complex, mediated by an interplay between dysbiotic microbial communities and aberrant immune responses within gingival and periodontal tissues. It is being recorded as public health problems. Objective: The aim of study is to detect Entamoeba gingivalis parasite in the oral cavity of patients with periodontitis and gingivitis by using molecular technique with singleplex polymerase chain reaction (PCR). Materials and Methods: A total of 100 patients with periodontal diseases (periodontitis and gingivitis) were enrolled in the current study. Samples of dental plaque were collected from each patient and stained with Giemsa stain and studied under a microscope. Six samples were selected to be examined by using singleplex PCR technique for the detection of 18S-the small subunit of ribosomal RNA gene (SSU rDNA) gene with 203 bp and comparing the results. Results: This result showed a high significant prevalence of E. gingivalis in dental plaque samples in patients with periodontitis and gingivitis. About 46 (46%) samples were positive by microscope examination. In comparison, six positive samples (3 men and 3 women) were selected for parasitic investigation by singleplex PCR and the results of PCR with specific primers designed for the detection of 18S SSU rDNA gene of (203 bp) showed positive results for the six samples. Conclusion: It was concluded that E. gingivalis had a prevalence among male patients than female patients. In addition, singleplex PCR is the technique of choice for the detection of the target sequence of DNA.

Type II Diabetic Mellitus (TIIDM) is a group of metabolic syndrome described by decline sensitivity of target organs to insulin and confirm or relative insufficiency of insulin excretion followed by electrolytes, water, protein, fat and other metabolic disorders, these defect result in diabetic patient to be susceptible to infect with intestinal parasites. The current study is conducted on 106 Iraqi participants,(67 with TIIDM patients and infected with parasite and 39 TIIDM without parasitic infection as control),aged (25 – 75) years, whom diagnosed by Consultant physician, according to the clinical signs and laboratory diagnosis HbA1c and hematological test (WBC, NEU, LYM, Hb ), during the period between November 2021 to March 2022. The practical part conducted at Kirkuk city in laboratories of Azadi Teaching Hospital (ATH). Two ml of blood sample were taken by intravenous injection and put in sterile tubes that contain anticoagulant EDTA and performed the following tests: HbA1c, Complete Blood Count (CBC). One hundred and six (106) participates (67 diabetic patients infected with parasite and 39 diabetic patients without parasitic infection (control) after the laboratory6 diagnosis, the result was: the ratio of white blood cell (WBC) and lymphocyte (LYM) increased, but depletion occurred in the ratio of Hb in TIIDM patient infected with parasite. This study proved that Blastocystis hominis is the most common type of parasite that has an effect on type II diabetic patients that affect hematological parameters, and Schistosoma mansoni has the least effect.

Irritable bowel syndrome (IBS) was a chronic functional gastrointestinal disorder with an estimated prevalence of 10% around the globe. (IBS) is a common characterized by recurrent abdominal pain or discomfort associated with altered bowel habit. Humans and animals gut contain intestinal parasites live in, and the intestinal wall is preferred for most of these parasites. Human intestine contaminated by many parasites, the most common of them are intestinal worms, Giardia, blastocyst and others. Method: The current study was conducted on 76 Iraqi participants suffering from Irritable Bowel Syndrome, and the diagnosis is made by the consultant physician, according to clinical signs and laboratory diagnosis (blood test (WBC, NEU, LYM, Hb), during the period from November 2021 to March 2022. Blood The sample (2 ml) was drawn intravenously with a sterile injection and then emptied in a plastic tubes containing EDTA anticoagulant and a complete blood count(CBC) test was performed. A stool sample was taken and examined under a microscope to see if the sample contained parasites or not. Result: We have noticed the presence of different types of parasites in 54 samples with irritable bowel syndrome, the most prominent of them Blastocyst hominis and Entamoeba histolytica and 22 samples that do not contain parasites, after the laboratory diagnosis , the result was that: the ratio of white blood cell (WBC) and lymphocyte (LYM) increased, but decrease occurred in the ratio of Hb for the IBS patient infected with parasite. Conclusion: This study proved that the highest rate of parasite infection was for Blastocystis hominis and the lowest percentage for Teania saginata which also affects the hematological parameters in patients with irritable bowel syndrome.

Oxidative Stress and Ascaris Lumbricoides Infection in Rural Children

Therefore, the aim of the present work is to detect the role of A. lumbricoides in an inducing oxidative stress in young children and their effect on the levels of some antioxidant enzymes. The current work was done among children aged 6 years to 14 years in Al-Hawijacity, Kirkuk, Iraq. 174 children used in current study at March 2019 to December2020. The feces were examined by the direct methodto detect A. lumbricoides. The presentworkexhibited that the number of infected children out of 174 is 23 (13.2%) children with A. lumbricoides. Otherwise, the outcomesshowed that the levels of Malondialdehyde (MDA), Glutathione (GSH), and superoxide dismutase (SOD)and catalase in an infected children show significant (P < 0.05) reduce compared with healthy children. So, the current study show that A. lumbricoides induced oxidative stress and causing disorders in antioxidant enzymes levels, which play important against the reactive oxygen species.

The effect of Annona muricata and Urtica dioica on killing the proscolex of Echinococcus granulosus

This study showed the effect of Annona muricata and Urtica dioica on killing the proscolex of Echinococcus granulosus parasite. The echinococcus cysts was isolated from the livers of slaughtered sheep in Mosul city. The vitality of the cysts was studied under the microscope using a 0.1% aqueous aucine. concentrations were used (10, 20, 30, 40, 50, 60, 70, 80, 90) µg/ml from an extract of Annona muricata and Urtica dioica plants to study its influence on the vitality number of the proscolex. These proscolex were treated with the extract for different time periods ranging from (0, 15, 30, 45, 60) minutes, the study showed a significant effect of Urtica dioica plant on killing the proscolex of Echinococcus granulosus. Flavonoids had a fatal effect on killing the proscolex in the concentrations (10, 20, 30, 40, 50, 60, 70, 80, 90) µg/ml for time periods ranging from (0, 15, 30, 45, 60) minutes. The 90 µg/ml showed a high effect on killing the proscolex. Alkaloids also showed a similar effect to flavonoids on killing the proscolex in the same concentrations and time periods, while the alkaloids were extracted from Annona muricata has a moderately effective on killing the proscolex in the same concentrations and time periods. While the cleucosides and flavonoids extracted from Urtica dioica did not show any effect in the same concentrations and time periods.

Phenolic compounds role in rat immunity changes that caused by entamoeba histolytica

The present study was designed to show the potential role of phenolic compounds against the toxicity of Entamoeba histolytica. The study used 20 adult male rats that distributed to four groups (each group consist 5 rats); control group that received normal saline, second group rat injected intraperitoneal with E. histolytica at dose 103 cyst/ml. third group rat injected intraperitoneal with E. histolytica at dose 103cyst/ml and treated with 100ug/ml of phenolic compoundsfor four weeks. Fourth group rat injected intraperitoneal with E. histolytica at dose 103cyst/ml and treated with 250ug/ml of phenolic compoundsfor four weeks. The results show high significant increased (P < 0.05) in levels of Interferon gamma (INF-γ) and Tumor necrosis factor-α (TNFα) in group that injected with E. histolytica compared with control group. About oxidative stress, Levels of Malondialdehyde (MDA), Glutathione (GSH) and catalase show high significant changes (P < 0.05) in group that injected with E. histolytica compared with control group. After using phenolic compounds, levels of INF-γ, TNF-α MDA, GSH and catalase in treated groups show non-significant changes (P < 0.05) compared with control group. The activity of camel milk to treated immunity changes that induced by Giardia lamblia in male rats

The current study was aimed to detect the activity of camel milk to treat the immunity changes induced by G. lamblia. 20 male rats using and divided into four groups (5 rats in each group); control group that received normal saline, second group rat injected intraperitoneally with G. lamblia at dose 7X107 cyst/ml. third group rat injected intraperitonealG. lamblia at dose 7X107 cyst/ml and treated with 0.5ml of camel milk for 2 weeks. Fourth group: rat injected intraperitoneally with G. lamblia at dose 7X107 cyst/ml and treated with 1ml of camel milk for 2 weeks. The results, after two weeks, show significant changes (P < 0.05) in concentrations of IL-2, IL-6, and IL-8 in the group that injected with G. lamblia compared with the control group. The results, after four weeks, demonstrated high significant changes (P < 0.05) in concentrations of IL-2, IL-6, and IL-8 in the group that injected with G. lamblia compared with the control group. After using camel milk, concentrations of IL-2, IL-6, and IL-8 in the group are treated groups (after two and four weeks) demonstrated non-significant changes (P < 0.05) with the control group. The present study was designed to show the potential role of Matricaria chamomilla flower extract against toxicity of Entamoeba histolytica. The study used 20 adult albino male rats that divide randomly to four groups (each group consist 5 rats); control group received ad libidium, group of rats administrated with 1X103 cyct/ml suspension for seven days, group of rats administrated with 1X103 cyct/ml suspension and treated with 50mg/kg extract for month, group of rats administrated with 1X103 cyct/ml suspension and treated with 100mg/kg extract for month. The results show high significant increased (P < 0.05) in levels of Alanine Aminotransferase (ALT), Aspartate Aminotransferase (AST), Alkaline Phosphatase (ALP), urea, creatinine and uric acid in rats administrated with 1X103 cyct/ml suspension. Oxidative stress factor in this group show significant increased (P < 0.05) in levels of MDA (malonedialdehyied) and significant decreased (P < 0.05) in levels of glutathione (GSH) and catalase compared with control group. While, after used M. chamomilla flower extract with Entamoeba histolytica, the results showed non significant changes (P < 0.05) in liver and kidneys function parameters. Also, MDA, GSH and catalase showed non significant changes (P < 0.05) compared with control group. It was concluded that flower extract has been potential role against toxicity of Entamoeba histolytica in adult male albino rats. © 2018 University of Baghdad-College of Science. Partial purification and properties of adenosine triphosphatase (ATPase) from liver fluke Fasciola hepatica.

The adenosine triphosphatase (ATP phosphohydrolase, EC 3.6.1.3.;ATPase) is a membrane -bound enzyme which transport protons across the plasma membrane using ATP as an energy source. The adenosine triphosphatase (ATPase ; EC: 3.6.1.3) was extracted from membrane preparations of adult Fasciola hepatica by chloroform treatment and purified by means of ammonium sulphate fractionation, gel filtration on sephadex G-200 and DEAE- Cellulose chromatography. The molecular weight was calculated to be 305.000 dalton by gel filtration. Kinetic experiments demonstrated a biphasic linear lineweaver - burk relationship (km=0.142 and 1.66 mM) thus revealing the existence of two substrate binding enzyme sites. In our study revealed that partial inhibition of Mg2+ dependent purified enzyme by oligomycin suggest the absence of mitochondrial ATPase in F. hepatica.
